The Neapolitan Mastiff is steady and loyal to his owner, not aggressive or apt to bite without reason. As a protector of his property and owners, he is always watchful and does not relish intrusion by strangers into his personal space.

Over the centuries, breeders of the Mastino in the Neapolitan area of southern Italy focused on breeding guards for the homes and estate. They created a breed that retained the giant size, heavy, loose skin, and dewlap.
The health problems of the Neapolitan Mastiff are due to two main factors. Firstly the Neo shares the problems of all large breeds - hips, hearts, and heat. Secondly today's Neos are decendants of a small gene pool used in their reconstruction.
In South Africa, the Kennel Club breed standard is followed. This is version 3, published in 1994. The Neapolitan Mastiff is exhibited in the Working Group. In FCI countries, the Neo is shown in Group 2 - Molossoids.
